I declare that I desire to operate my business on Penn College's campus, and further, by signing I acknowledge my understanding that my participation in the activity/event could result in injury, death, damage, or loss to my person, my property, other persons, and other persons' property. I hereby voluntarily release, indemnify, and hold harmless Pennsylvania College of Technology, its employees, officers, directors, and agents from any liability for any injury or damage resulting from my participation in the identified event/activity.

A Certificate of Insurance should be requested from your business office or insurance agent and then forwarded on to me via email. (This link provides information on required limits and general rules.) The date of your event on campus should fall within the policy dates listed on the certificate, and Penn College must be listed as an additional insured.
