Disability and Access Resources will review and consider all pieces of documentation; however, it is best to provide disability-specific assessments which have been used by qualified professionals to support the diagnosis and include interpretation of results (a description of the functional limitations created by the disability). Examples of these types of assessments include, but are not limited to:

*Examples of aptitude and achievement testing: Wechsler Adult Intelligence Scale, Woodcock-Johnson Cognitive Ability, Stanford-Binet Intelligence Scale, Kaufmann Intelligence Assessment Test, Wechsler Individual Achievement Test, Woodcock-Johnson Test of Achievement, Scholastic Abilities Tests for Adults, Kaufman Test of Educational Achievement, etc.

**Examples of Behavioral Rating Scales: BASC, Conners, Vanderbilt, etc.
