EMT Registration

I fully understand that the Pennsylvania Department of Health requires that all individuals participating in an Emergency Medical Services certification examination, self-disclose any and all criminal convictions.  Individuals failing to supply complete and factual criminal history documentation may be denied certification.  Furthermore, I understand that any tuition paid for this course is non-refundable in the event that the Pennsylvania Department of Health does not allow participation in the Emergency Medical Technician certification exam.

My tuition will be refunded in full, providing I drop the course seven (7) days prior to the first day of class.  I understand that no refunds will be issued in the event that I drop after this date, or low attendance – 12 or more hours missed - warrants withdrawal from the class.

I hereby grant absolute rights and permission to Pennsylvania College of Technology to take photographs, video, and/or audio that may be used and distributed by the College for educational, promotional, publicity or advertising purposes.

I understand that my words, voice, and images may be distributed in (but not limited to) any or all of the following formats: Internet, videotape, audiotape, television or radio broadcasts, newspapers, magazines, printed materials, CD-ROM, DVD, e-mail messages or other forms of publicity and promotion for Penn College.

I understand that Penn College holds the copyright to my words, voice and image, and that I will not now, or at any time in the future, receive any type of monetary compensation from Penn College for use of my words, voice or image.
