All incoming Penn College students are required to meet the CDC and American College Health Association immunization guidelines prior to starting classes. (Penn College Health Services provides immunizations on a fee-for-service basis.)

Required Immunizations

All students are required to complete the following immunization series

  • Two doses of Measles (Rubeola), two doses of Mumps and one dose of Rubella OR
  • Two doses of the MMR vaccine OR
  • Results from a blood test showing immunity

The first dose must have been given on or after the student's first birthday. Dose two must have been given at least four weeks after dose one.

One dose of Tdap (tetanus toxoid, reduced diphtheria toxoid and acellular pertussis, NOT to be confused with DTap or Td) vaccine is needed for immunity. A Td vaccine booster is also needed if it has been greater than ten years since the Tdap was administered.

Hepatitis B is transmitted through body fluids, including blood. Three doses of Hepatitis B vaccine are needed for immunity. Doses one and two must be administered at least four weeks apart. Dose three should be at least six months after the first dose and eight weeks after dose two. A blood test may be used to confirm immunity.

Two doses of varicella (chicken pox) vaccine are needed for immunity. The doses must be administered at least 28 days apart. OR a blood test confirming immunity if you have had the chicken pox disease.

Also known as Menveo, Menactra or MenQuadfi. This vaccine is effective against the most common meningococcal infections caused by serogroups A, C, W, and Y.

One dose of meningococcal conjugate vaccine administered at age 16 or older is required. Age 23 or older exempt.

All international students arriving in the United States within the past five years from countries with endemic Tuberculosis must meet Penn College's Tuberculosis (TB) screening (PPD skin testing, Quantiferon TB Gold or T-spot, and/or chest X-ray) requirement. This requirement is in keeping with current CDC guidelines.

The Tuberculosis (TB) Screening Requirement (for international students) cannot be declined.

Recommended Immunizations

All students are recommended to complete the following immunization series

Also known as Bexsero or Trumenba. This vaccine is effective against most of the more common meningococcal infections caused by serogroup B. These vaccines are given in a series of either two or three injections and are not interchangeable.  Age 23 or older exempt.

Submitting Your Immunization Records

All incoming undergraduate and graduate students must submit proof of immunizations prior to their arrival on campus. Use the two-step process outlined on this page to submit your information.

  1. PRINT and TAKE the Immunization Verification Form to your health care provider for completion.
    The Immunization Verification Form MUST contain the required immunization information along with the provider's signature, title, date, address and ORGANIZATIONAL STAMP. A copy of your immunization records can accompany the Immunization Verification Form.
  2. Enter your immunizations and upload documentation of your immunization records from your provider to College Health Portal.
    All records must include your full name, date of birth, and student ID number.

*Records are due on or before the date you move on campus and/or start classes, whichever comes first.  You will not be permitted to move into the residence halls without Meningococcal MCV4 documentation on file.  Failure of any student to provide immunization records will result in a scheduling hold.

Documentation of your immunizations require the signature of your medical provider. You can download the forms below to take to your medical provider. (Medical provider forms are also accepted for documentation. Please ensure your full name, date of birth, and student ID number are on the forms.)

More Information

  • Based on a recent Pennsylvania statute for educational institutions, you have five days to submit documentation. Penn College will evaluate compliance weekly.
  • If you have a medical contraindication to any of Penn College's immunization requirements, submit a letter of explanation, signed by both you and your medical provider.
  • Waiver forms are available to those students who have a strong personal objection to vaccination with one or more of the required vaccines. Please be aware that if a communicable disease outbreak occurs, those who are not properly vaccinated or have a waiver due to a personal or religious belief will be the first to be asked to leave campus for their own safety.
