Physicals
Limited Physicals
Limited physical examinations (i.e. work, driver’s license) are available
through Student Health Services. Physical forms and proper identification are
required. There will be a fee for this service.
Athletic Physicals
Sports physicals for Penn College student athletes will be offered by sport on designated dates. All physicals will be conducted in the Student Health Services Office, Bush Campus Center, Room 150.
Students should bring the following to their physical:
• Completed Pre-participation Exam Form (PDF)
• $25 - cash only
